چکیده
مقدمه:
مولتیپل اسکلروزیس (MS) یک بیماری مزمن سیستم عصبی مرکزی است که اغلب با مشکلات شناختی همراه است. این مشکلات ممکن است از طریق افزایش اضطراب، افسردگی و دشواری در تنظیم هیجان، بر خواب بیماران اثر منفی داشته باشند. هدف پژوهش حاضر بررسی نقش میانجی اضطراب، افسردگی و تنظیم هیجان در رابطه بین مشکلات شناختی و اختلالات خواب در بیماران MS بود.
روشها:
پژوهش حاضر بهصورت مقطعی و با استفاده از مدلسازی معادلات ساختاری انجام شد. تعداد ۳۰۲ بیمار مبتلا به MS از مراکز درمانی شهر اصفهان در سال ۱۴۰۲ بهصورت هدفمند انتخاب شدند. ابزارهای مورد استفاده شامل پرسشنامههای NOCUG، PSQI، HADS و ERQ بود. دادهها با نرمافزارهای SPSS-21 و AMOS-4 تحلیل شدند.
یافتهها:
مشکلات شناختی اثر مستقیم معناداری بر اختلالات خواب داشت(β = -0.304) . همچنین افسردگی (β = 0.326) و ارزیابی مجدد (β = -0.373) با اختلالات خواب رابطه معناداری داشتند، اما اضطراب (β = -0.115) و سرکوب (β = 0.047) اثر معناداری نداشتند. مشکلات شناختی بهطور مستقیم بر افسردگی (β = -0.78)، اضطراب (β = -0.653)، ارزیابی مجدد (β = 0.779) و سرکوب (β = -0.314) اثر معنادار داشت. همچنین اثر غیرمستقیم مشکلات شناختی بر اختلالات خواب از طریق افسردگی (b = -0.053) و ارزیابی مجدد (b = -0.061) معنادار بود.
نتیجهگیری:
افسردگی و ارزیابی مجدد نقش میانجی مهمی در رابطه بین مشکلات شناختی و اختلالات خواب دارند و میتوانند بهعنوان اهداف مداخلهای مؤثر برای بهبود خواب بیماران MS مدنظر قرار گیرند.

چکیده English
Background:
Multiple sclerosis (MS) is a chronic disease of the central nervous system that is often associated with cognitive impairments. These impairments may negatively affect patients' sleep by increasing anxiety, depression, and difficulties in emotion regulation. The present study aimed to investigate the mediating role of anxiety, depression, and emotion regulation in the relationship between cognitive problems and sleep disturbances in patients with MS.
Methods:
T his cross-sectional study was conducted using structural equation modeling (SEM). A total of 302 patients with MS were purposefully selected from treatment centers in Isfahan in 2023. Research instruments included the NOCUG, PSQI, HADS, and ERQ questionnaires. Data were analyzed using SPSS-21 and AMOS-4 software.
Results:
Cognitive problems had a significant direct effect on sleep disturbances (β = -0.304). Depression (β = 0.326) and cognitive reappraisal (β = -0.373) also showed significant effects on sleep disturbances, while anxiety (β = -0.115) and suppression (β = 0.047) were not significant. Cognitive problems had significant direct effects on depression (β = -0.78), anxiety (β = -0.653), cognitive reappraisal (β = 0.779), and suppression (β = -0.314). The indirect effects of cognitive problems on sleep disturbances through depression (b = -0.053) and cognitive reappraisal (b = -0.061) were also significant.
Conclusion:
Depression and cognitive reappraisal play significant mediating roles in the relationship between cognitive impairments and sleep disturbances and may serve as effective intervention targets to improve sleep quality in patients with MS.
