Background
Psilocybin is an indoleamine prodrug found in more than 200 mushroom species and is being investigated as a potential psychiatric intervention. However, Iranian evidence regarding taxonomy, patterns of use, clinical management, and regulation remains limited.
Methods
This narrative review searched PubMed/MEDLINE through the PubMed interface, Google Scholar, and NoorMags (Noor Specialized Journal Website) in English and Persian through 17 July 2026. Search terms addressed psilocybin, psilocin, Psilocybe, magic mushrooms, psychedelics, clinical trials, toxicity, poisoning, and Iran. Relevant regulatory documents and conference abstracts were reviewed manually.
Findings
Approximately 140 psychoactive species of Psilocybe have been described worldwide, and Iranian mycological surveys have reported P. cyanescens, P. serbica, and P. atrobrunnea. Clinical studies, including phase II trials in depression and smaller studies in cancer-related distress and substance use disorders, have reported clinically meaningful and sometimes sustained symptom reductions. However, small samples, heterogeneous psychological support, expectancy effects, and difficulties maintaining blinding limit the certainty of these findings. Common acute adverse effects include headache, nausea, anxiety, dizziness, and transient blood-pressure elevation. Persistent serious psychiatric events appear uncommon in screened and monitored settings, although clinically important risks remain for susceptible individuals.
Conclusion
International evidence suggests that psilocybin has relatively low acute physiological toxicity and potential therapeutic value when administered in controlled settings with psychological support, and professional training and clear clinical governance is required. Priorities include local research, ethics-based professional training, culturally adapted education, toxicology preparedness, and regulatory review.
